Our 15 bedded hospice provides care for adults and young people with cancer and other progressive, life threatening illnesses. We provide specialist care at home with regular contact by Nurse Specialists and Doctors to assess and address the patient and carers needs and problems. We provide an individual programme of care for patients, through two patient day groups. Our team of Specialist Nurses and Consultant at Darent Valley Hospital and Consultant at Queen Mary's Hospital give advice and support for adults, and their families, who have been admitted to hospital with cancer and other progressive, life limiting illnesses. Our children's team provides care for children with life threatening and life limiting conditions, from new born to young adults. Transition from children's services to adult services can be a difficult experience, both for the young person involved and their family. Our work ensures that this group of young people have appropriate care and support available
